The demands of high patient volumes, also typical in New York, can strain those “weak links” to a breaking point.  A busy Emergency Room Attending may be tempted to delegate too much to an inexperienced resident. A nurse in a busy post operative unit might not take the time to check a medication dose.  Poor communications can also lead to poor care, even at “good” hospitals –such as in one of our cases in which a patient’s respiratory problems during surgery were not communicated to the post-anesthesia team, leading to a “cardiopulmonary arrest” and severe injury.
